Could Someone Help Me With Finding A Vacc Exemption Form For My Grandmother That's Been In The Nursing Home Since 1999!!!!!!

She Do Not Want To Take It And I Stand With Her Decision!!!!!!(We Don't Want To Alter Our DNA)

I Previously Search This Website Under Information And It Seems The Forms That Are Already Provided Are For Children Or Someone Who Are Attending School!!! Although I do find the info very useful, I'm really trying to hemp my grandmother in the nursing home facility!!!!
